Course Description

The Indigenous Care Coordination Program is a comprehensive program designed to equip participants with foundational skills for providing wholistic, integrated support to clients, families, and communities. The program emphasizes a wholistic approach to care, ensuring that participants develop practical skills to address unique needs and improve service accessibility for Indigenous populations. Key topics include navigating Non-Insured Health Benefits (NIHB) and Jordan’s Principle, empowering participants to effectively coordinate resources and advocate for equitable healthcare access. Participants will engage in real-world scenarios, case studies, and role-playing exercises to build practical skills in culturally safe care coordination. This structure is designed to foster both individual skill-building and collaborative problem-solving, ultimately enhancing participants’ confidence in providing integrated, culturally responsive care.

Course Instructors

  • Highly experienced facilitators with expertise in Indigenous healthcare policy and procedure development, cultural competence, trauma-informed care, and accreditation standards will lead the workshop. This course will be taught by an Indigenous Registered Nurse alongside a Registered Nurse who is an ally to the Indigenous community.

Program Objectives

Upon program completion, learners will have gained knowledge related to:

  1. Understanding Indigenous Health Contexts: Acquire a comprehensive understanding of the historical and contemporary factors influencing Indigenous health.
  2. Developing Wholistic Care Coordination Skills: Learn and implement relevant care coordination and case management strategies, ensuring the needs of the client, family and community are supported.
  3. Indigenous Case Management Strategies: Learn practical effective case management strategies tailored to Indigenous contexts, integrating traditional knowledge, community resources, and holistic health and well-being approaches.

Session 1:  Foundations of Care Coordination within Indigenous Health

Objectives: 

  1. Explore the principles and competencies related to Care Coordination.
  2. Understand the role of Care Coordination in supporting holistic approaches to care.
  3. Identify specific socio-economic determinants of health that impact communities.

 


Session 2: Principles of Culturally Relevant Care Coordination

Objectives: 

  1. Explore principles of culturally relevant care coordination.
  2. Develop strategies for effective communication.
  3. Identify various local, regional, and national programs and supports.

 


Session 3: Practical Skills for Indigenous Care Coordination

Objectives: 

  1. Apply relevant approaches to real-world care coordination scenarios.
  2. Support the development of individualized care plans with Indigenous clients and families.
  3. Navigate various local, regional, and national programs and supports.  

 


Session 4: Consolidation and Evaluation

Objectives: 

  1. Review of reporting processes and data collection.
  2. Consolidating Care Coordination approaches; reviewing skills and scenarios.
  3. Course Evaluations and Next Steps.
